Before You Begin
Requirements
You must have a Master Admin login or a staff login with the following permissions:
- Reporting - Can Access Reports (required to view any reports)
- Reporting - Can Access Staff: Disciplinary
Important Notes
- Reports can be retrieved for up to 10 years from the current date.
- Larger date ranges may take longer to generate and download.
- Reports are not customizable.
- This report reflects data in real time.
- There are two ways to access disciplinary records:
- View all Open Disciplinary Actions
- View Disciplinary Actions by Date Range
Option One: Open Disciplinary Actions
Navigate to Reports > Staff > Disciplinary Actions.
Click the button Open Tab and select Run Report to display all of the open records.
The online report will display: Date Entered, Status, Employee User ID, Employee Name, Termination? and Incident Detail.

Option Two: Disciplinary Actions By Date Range
Another option is to search for disciplinary actions during a specific time period. To use this feature click on the Date Range tab. Select a preset date range or custom date range using the calendars to the left. You can also select a discipline type from the filter drop-down. To generate the report, click on Run Report.
The online spreadsheet will display: Date Entered, Status, Employee User Id, Employee Name, Job Title, Entered By, Supervisor Name, Supervisor Job Title, Incident Type, Incident Start Date, Incident End Date, Incident Detail, Policies Violated, Counseling, Verbal Warning, Written Warning, Suspension, Termination and Other Action.
