This article will guide you through the steps on how to create a health history form with Custom Forms to use as a Presentation. This allows to create a signable form that then saves as a pdf.
Step 1. Create a Custom Form
Navigate to Setup > General > Custom Forms. Continue by completing the available fields, making sure you select Health History as the Type of Form.
Click Create Form to save.
Step 2. Health History Form Settings
To create the content for the health history form, you will be adding form elements to a document using the club forms editor. The eight elements that can be added are:
- Text Block
- Client Info (limit 1 per form)
- Confirmation
- Multiple Choice
- Signature
- Question
- Section Header/Separator
- How Did They Hear About Us? (limit 1 per form)
Select from the drop down the Element Type you are interested in using. In the example below we used Section Header/Separator, Client Information (gather basic details such as name and contact), Multiple Choice (provide questions and answers that you need), Question (provide a blank field they can answer specific questions you have) and Signature (gather their electronic signature).
Step 3. Create a Presentation & Tie Health History Custom Form
Once the Health History Form is completed, it will need to be added to a presentation in order to show to the user and gather their information. Think of a presentation as a slide show that can be created to assist you in presenting everything from waiver of liability forms, sales, or in this case, the health history form.
Navigate to Setup > Leads > Presentations. To start creating a presentation enter in a Presentation Title and Description. Click on Create New Presentation Ready To Edit. This will create the presentation below. Click on the presentation name to open up the presentation setup.
On the left side of the screen, select Click to Add New Slide. This will prompt a list on the right side of the screen. Select Custom Form.
Type the Step Header Text (title of the slide when presenting to the user) and Select the custom form from the drop down (in this case, your health history form).
Click the Update Step button to save your changes.
