Section 1: Sub-Merchant Information
This section is regarding the legal entity that your account will be set up under. Most of the fields will be filled out for you by the merchant specialist working with you on your account. The main fields that you need to focus on are blocked out in red in the image below.
Date of Formation & Approx. Years in Business
The approximate date that the legal entity was approved & the amount of years/months that have passed since that date.
Business Address
This field will be the address you want tax documents sent to. It can be your personal address, the studio’s address, or the address the entity is registered under if it isn’t either of those addresses. Please enter the full address as it is shown in the screenshot above.
Website URL
This field may already be filled out for you. If not, please enter the URL for your business’s website.
Business Type
For this field, please select the box that applies to your legal entity.
Primary Contact/Business Phone
Please enter the desired phone numbers.
Section 2: Transaction Information
For this section, we are looking for ballpark estimates of what you think the studio will bring in. You should have these numbers from your business plan but if not, again we are just looking for estimates for all but the Max. Ticket fields. If you’re not sure you can enter N/A but it is preferable that an amount is entered.
For clarification here is what the terms are referring to:

Volume – Dollar amount of percentage of total sales.
Example: If total monthly sales is $100 & $80 of that amount is ACH the Monthly ACH Volume would be $80 & the Monthly Card Volume would be $20 - Sales – Dollar amount from the sales within that period
- Ticket – Dollar amount of a single sale
- Max. Ticket – The maximum dollar amount for a single sale
Section 4: Owner Information
This section is where you’ll enter personal information for all owners listed on the previously filled out LER. Most of the fields will be filled out for you by the merchant specialist working with you on your account. The main fields that you need to focus on are blocked out in red in the image below.
Address (Home or Business)
Please enter your desired address as it is shown in the screenshot above.
SSN or FEIN
Please enter your SSN or your SIN if you are Canadian.
If your legal entity is owned by another entity & you want that documented on the SMAA this is where you would do so. Please put the entity’s information in the Control Owner section. If the owner you are entering is a person you need to enter their SSN. Only enter an FEIN if the owner that is being entered is another legal entity.
Please note. WorldPay requires at least one person to be on the account. We cannot enter only the legal entity as an owner. We would need at least one human owner & their SSN.

Common Questions
There isn’t a section for me to enter my co-owner’s information, how do we get their information on the form?
Each owner is sent their own copy of the DocuSign to complete. Anything outside of your designated fields will appear blank. Once everyone completes their copy, we (ClubReady) will receive one completed form from DocuSign. This is what we will use to enter into WorldPay.
I’ve filled out the SMAA, does this mean I will start being charged fees?
Transaction fees do not start until transactions start. Software fees and/or other fees start when Presale starts or Conversion Go Live.
Will it really take 5 days to get my account setup?
Not normally, no. This time frame is mentioned to set expectations in case we have a high volume of accounts at that time or if the account goes into manual review & needs to be looked into by the WorldPay team.
What is manual review?
It means that something on the application raised a flag in WorldPay’s system & it needs to be review by one of their agents. In most cases it’s a false flag & they will release the account in a few days. If it’s not a false flag normally it is just that information was entered incorrectly. If that’s not the case though & they require more information we will let you know what’s needed.
